PATIENT PORTAL REGISTRATION AND INSTRUCTIONS
To prepare for your upcoming exam or surgery, Tylock-George Eye Care requests that you either complete your medical history online using our Patient Portal before your appointment or by downloading the appropriate registration form, and bringing it completed and printed out to your appointment.

Online Registration Through Patient Portal
As soon as your surgery has been scheduled we recommend that you enter your medical history online. Online registration will allow our Pre-Surgical Assessment nurse will be able to access the information before your appointment. This information will assist the nurse in organizing and documenting your complete medical history to prepare for your surgery.
Be sure to have the following information available before starting pre-registration with our online Patient Portal:
- Your health insurance information.
- The names, addresses and phone numbers of your physicians.
- A list of all medications you are taking, their dosage and frequency.
- A list of surgical procedures you have ever had
If you are a new patient or have not completed your annual paperwork since February 2018, please follow the instructions below:
STEP 1: Register For The Portal
Once there, if you have not created a Patient Portal ID please select the link that says “First Time Patient Portal Access” and continue. Fill out your general demographic information (name, address, etc.) and click to Continue. The information you provide should match the information we have used to set your appointment by phone.
STEP 2: Use the Temporary Portal Key
Check your email. We email a temporary key that will allow you to finish your registration on the second page of the portal. We send out the email prior to you coming in for your appointment and each email has a randomly generated “TEMPORARY PORTAL KEY”.
